Hello,
I would like to design the Contact form a bit differently, is it possible?
right now there are 3 areas:
Name & Surname / e-mail / message,

I would like to add some areas and maybe check boxes.
For example: Company / phone number / Fax / Country

Right now it looks like that:
http://daphneornertest.com/?page_id=147

and I would like to add areas like here:
http://smartrider.co.il/contactUs.asp

How can I do that?

Hello,

In this case you need to edit the wp-content/themes/legenda/contacts.php file to your needs.
Also as an alternative you can try to use “Contact Form 7” plugin to build custom contact forms.
