1) There could be few reasons of this issue and I can’t tell you which one is in your case without link to page:
a) Position of the content in the Row settings http://prntscr.com/qq61pf
b) Additional padding of the column or element http://prntscr.com/qq626c
So, provide us with link to page with this issue.
2) Because image is resized depending on the block width. If you need just add the line in 2px use either border for the text/heading element http://prntscr.com/qq64i9 or Separator WPBakery element http://prntscr.com/qq64y2

1) Position of the content in the Row settings http://prntscr.com/qq61pf Use Top instead of the Middle.
2) I don’t see the mentioned issue with the images https://gyazo.com/32ca88a93e24da7372d3e1409fb6b449 but I can just suppose that it caused by the resize. Anyway, if you want to avoid this issue use either one of the ways I described you before or hr tag, for example

Regarding no.2, can you see the upper one is bolder? And the second one is slimmer?
