Hi

You can chagne the parameters by going to Dashboard > MC4WP and set your mailchimp form according to your requirements.

Follow the MailChimp plugin documentation to set up the plugin and find information about main settings https://www.mc4wp.com/kb/

Read theme documentation to get the custom styles for the mailchimp form https://www.8theme.com/documentation/xstore/plugins/mail-chimp-form-custom-styles/
