I have no idea how or when this happened, but when customer adds to a cart, goes to checkout, enters billing address, shipping, etc. then places order – that section goes empty thus taking away the ability to order.
It explains why we haven’t received any orders for the past couple weeks… so I appreciate your help!

Some plugin is causing the issue.
Please deactivate them one by one to find it out.

Yep, glad it was simply a plugin. Apparently W3 Total Cache plugin was affecting the checkout…
