Customize Contact Form Plugin based to Cookies Ley

I need to add an explicit acceptance botton in my contact form page, belong to Cookies Ley dispositions:

This is the code that I want to add:

but I don’t know in which .php file and in which point I have to add it?

Can you pass me any info about?

Please clarify where you found the shortcode [acceptance Consenso]. It doesn’t work.

I didn’t remember exactly, probably in a wordpress blog, but it’s not correct (maybe because if it’s specificly for plugin Contact Form 7).

Which other shortcode can let me to insert a checkbox?
What can you tip me?
