How to configure : subscribe button asking to select at least one in list

Hello Support.
I want to know the flow of subcribe :
(1) Currently in the footer area, upon entering an email to subscribe it is asking to “Please select at least one list”, I am not sure why it is so and how/what to fix.

(2) After successful subscription how to make template and send newsletter using mailchimp

Please suggest.

The form was created using Mailchimp for WordPress plugin. Please read documentation of the plugin – https://kb.mc4wp.com/#utm_source=wp-plugin&utm_medium=mailchimp-for-wp&utm_campaign=plugins-page

I will go through the stated document.
however, currently the form itself not showing.
Once I click subscition button it shows only a message and no form comming up.
Could you suggest any instant way/steps.
