Hello,

Thank you for using our theme.

You can turn on/off links in the header top bar in Theme Options -> General -> Header Settings.

Add any content to prefooter area in Appearance -> Widgets.

Go to Mailchimp > Forms > and compose your form with fields you need. Here is an example of fields that we use http://prntscr.com/bwfn4n
