Having issues with newsletter pop up, does it require mailchimp plugin? if so please advise process?

Text and layout is fine, need the code for box to type email addresses and sign up button also.
How do I change what page the pop up appears on? I just want it to auto pop up on home page and when clicking link in top bar however it pops up on other pages.
