    Hey I am trying to add this form to our register page, so our customers must fill out and submit this form for approval before they are allowed access to prices on our site.

    I have the code and PDF file, just not sure how to accomplish this task.

    I know I have to put the file in the cpanel and the code on the specific page….but can you help me determine where to put these?

    Would you recommend using this:

    <object><embed width=”700″ height=”930″ src=”
    myfile.pdf” type=”application/pdf”></object>

    or

    <object data=”myfile.pdf” type=”application/pdf” width=”100%” height=”100%”>
    <p>Alternative text – include a link to the PDF!</p>

    Did you created that form on-line?
    Do you have any instructions how it should be imported to the site?

    No, I created that form through Adobe Acrobat.

    My friend who does code, wrote those codes for me. But I am still unsure as to where I need to put those codes to get this form on my site.

    Do you have any recommendations on an online form that will allow our customers to fill out for a credit credentials for a business account and then that info be sent to our email for review?

    I ended up using a third party form creator, which I was able to upload the form and put it online!
