Some text boxes not showing - by stickerpix

The box on the forgot my password form for the password entry does not show up. Is there a way to target all of those types of text entry boxes and make sure that they always appear?

Hello,

Please provide screenshots and temporary wp-admin access.

Create an account via Dashboard (Users > Add new) with administrator role http://prntscr.com/s3rc9m > provide us with username and password via the Private Content area http://prntscr.com/s5mao7 You can use any email for it.
